Introduction and importance: Rhabdomyosarcoma is the most common pediatric soft tissue sarcoma. It constitutes 4 – 6% of all malignancies in childhood. The botryoid variant a type ERMS that malignancy female genitourinary tract. vagina usually occurs early childhood, whereas uterus later age. Since 1959, only 7 cases uterine during infancy have been reported. Case presentation: A 15-months-old girl was admitted to our ED for three reddish, smooth, glistening, polypoidal masses protruding from her vaginal introitus 9 days. lesions had suspicious “grape-like” appearance tended come out through ostium. pelvic US examination on patient showed an inhomogeneous aspect uterus, which appeared be occupied stretched by iso-hypoechogenic solid tissue. Additionally, there were some hyperechogenic spots inside as well evidence vascular signals color doppler. surgical biopsy revealed malignant mesenchymal tumor consistent with diagnosis embryonal rhabdomyosarcoma (ERMS), variant. Clinical Discussion: completed first cycle chemotherapy ifosfamide, vincristine, dactinomycin (IVA). When she 18 months old, underwent hysterectomy, removed entire mass Pfannenstiel approach. postoperative course presented no complications. Follow-up visits scheduled quarterly. Conclusion: Botryoid not missed infant. rapid treatment are necessary, clinicians should aware its presentation assure optimal management. Highlights: Rhabdomyosarcoma (RMS) sarcoma 4–6% occur head neck.RMS contrast at age.In this case RMS occurred very young age; since infant reported.It aggressive form associated poor prognosis infant.
